Fever and Seizures

In Ward Putrajaya General Hospital

1) This is my experience as a father to a little daughter nicknamed Lalin'.  A story about fever and turn to seizures. A worry about life. I hope this would be useful for the first-time entitled parent.

2) For the beginning, let we understand what is seizure and how it occurs especially to a son / daughter from 6 months to 6 years old. I did some research on that and it would be easier for us to understand if we create some Q&A. Let's check it out!

a) What is seizure?
- seizure occurs when the electricity flow in the brain burst and out of control. As the consequences are as follow:

> muscles cramped
> eyed up
> foaming mouth
> tetanus
> unconscious

b) What is fever of seizures?

> seizure occurs when body temperature is over 38C
> it usually occurs at the first or second day of fever
> the age is from 6th month to 6th year.

c) Reasons to fever of seizures?
-Body high temperature caused by:

> upper respiratory infections
> ears infections (otitis media)

d) Would it repeat?

> yes, it would. 30% possible

e) Did the fever of seizures affect brain?

> No,unless it happened more than 5 minutes

f) Was it because of gents?

> Yes especially when families experienced it

g) How did you recognize fever of seizures?

> Body high temperature
> Body and Legs cramped
> Might be collapsed
> Foaming Mouth
> Couldn't stand to pee
> Slept and unconscious. Weak when conscious

h) What should you do while:
     i) Fever
       - Lighten clothes
       - Wet body with clothes
       - Feed paracetamol as required by
          Experts
       - Drind much plain water
       - Put them at the comfortable places
         that have good air flow

    ii) Seizures
       - Cool down yourself
       - Lay left your daughter /son
       - Put away dangerous things
       - Do not feed anything into mouth
       - Do not leave your daughter / son
          alone
       - If the seizures occurred more than 5
         minutes, get doctor at the nearest
         Hospital.

3) Owh, truly I was so surprised with the first time I experienced that my daughter was attacked to seizure. A story began when I decided to go holiday at Melaka but change plan to go Port Dickson while we were trapped in congestion. As usual, we were burn out of the sun heat, I did my water activities such as banana boat and swimming. At 6.30 pm we departed home from PD. I noticed that my daughter was not stable and seemed unhealthy. On the way to go home, my daughter was attacked to seizure in the car. I didn't know what to do, I dropped by at R&R Dengkil near Putrajaya. I hugged her tightly and pray that everything fine and under control. I saw she cramped with straight leg and hands for about 10 seconds. My wife screamed loudly when we both experienced it for the first time.

4) Fortunately, nothing serious happened. I brought my daughter to Putrajaya Hospital and my she was admitted to ward for further observation because they worried if the seizure repeat. I obeyed what the doctor said due to their medical expertise. A night in the hospital made me headache with no enough sleep and rest. However, I was ok to see my daughter's fine. With the serious observation from doctors and nurses, they fed medicines and checked body high pressure every three hours. I was satisfied. On tomorrow, my daughter seemed healthier and she messed around to go home. I asked permission from doctor to discharge her and the doctor permitted with condition: if something bad happen, they will not responsible. I convinced them, everything would be alright and anything happen hospital will be my first place to heading. Allah wills, nothing happen.

5) We both couldn't forget the moments when she was attacked seizure. It was terrible. That's our lesson learnt and would never repeated anymore. The doctor said, if she's attacked for the first time, it might repeat. So, we have to prepare medicines and anything necessarily. We hope, that's the first and the last.

6) I hope this would be a reference for those who are the first time entitled parents. Remember, our children are our responsibility. Anything happen to them will affect us as well. They are everything for us.

COVAC AT PANGKOR ISLAND

1) We need to allocate some times with our family as well as to delighting children upon their request. My children asked me to go holiday to have fun in beach. Then, Pangkor Island was my choice as we hadn't been there yet since we stayed at Teluk Intan, Perak. The distance is not too far but we have to take a ferry to reach that island.

2) 1 planned to enjoy a short vacation from 26 June until 28 June 2020 which is 3 days and 2 nights. I booked chalet and I also rented a car as to go around the island visiting interesting places.
 
3) My children enjoyed with water activities at the beach of Teluk Nipah, Pangkor. We filled our times mostly at the beach. We did no Island Hopping as well as snorkeling and islands to islands boating because I have a baby and it was not suitable for that.

4) On 26 June, we departed from home in Teluk Intan to Lumut Jetty. Tentatively as the following:

a) 26 June - Departure to Lumut Ferry. Parked car at Parkir Building rated RM15 per day. Take Ferry to Pangkor Island at 1pm. Arrived Teratak Chalet at Teluk Nipah at 2.30pm. At 4 pm jump into sea bathing!;
b) 27 June - Round Island. We went around the island and do visit some interesting places such as Dutch Fork, Floating Mosque, Shops selling seafoods ( salted anchovies and many other products seafoods base). At evening, Bath again at the beach; and
c) In early morning, take time to bath again and by 12 noon we checked out to go home.

5) This is so called COVID-19 + vacation ( COVAC). Our short vacation seems meaningful to family bonding times and at the same time for children's happiness.

6) Pangkor Island is an expensive island for local tourists. The Island was just launched as Freex Tax Island but the prices they offered are otherwise. I do not recommend others to have vacation there but if for getting experience for the first time - it's ok! I shocked with Pangkor anchovies sold there with so expensive RM100/kg. The seafood at restaurants served not fresh and expensive. For me, the price should be cheaper because it's in an island! Moreover, chocolates sold there also very expensive. I don't know how the island was gazetted as free tax island. The journey to become as good as Langkawi is still far to reach the par!

7) Having a vacation during RMCO COVID-19 is totally different because we were asked to follow SOP all times. However, sometimes self -regulation to stay focus and in line with SOP to avoid the spread of the virus is much important. We didn't go out in crawded places such as night markets. We must have self consciousness to help the government of Malaysia to stop the spread and become uncontrolled if we take it as granted. We should be grateful because we are now free to go holiday anywhere locally.

8) However, we enjoyed for the first time experience on that Island - that's more important!

"BERKAT BERJASA" - A BIG ROLE IN HELPING PEOPLE DURING MOVEMENT CONTROL ORDER (MCO) DUE TO COVID-19

1) The figure revealed by World Health Organization (WHO) - there have been more than 7.8M confirmed cases of COVID-19, including more than 430 thousands deaths while in Malaysia recorded more than 8.4 thousands positive  cases  and 121 deaths as of 15 June 2020. The numbers is  still increasing worldwide.

2) Cronologically, our beloved country, government of Malaysia led by Tan Sri Muhyiddin Yasin (PM-8) has made an official announcement on 16 March 2020 promulgated the implementation of Movement Control Order (MCO) from 18 March 2020 under the Prevention and  Control of Infectious Diseases Act 1988 and the Police Act 1967. The order included the following restrictions:

✓ General prohibition of mass movements and gatherings across the country including religious, sports, social and cultural activities. To enforce this prohibition, all houses of worship and business premises would be closed, except for supermarkets, public markets, grocery stores and convenience stores selling everyday necessities. Specifically for Muslims, the adjournment of all religious activities in mosques including Friday prayers would be in line with the decision made on 15 March 2020 by the Special Muzakarah Committee Meeting of the National Fatwa Council;
✓ Sanctions covering all Malaysians travelling abroad. For those who have just returned from overseas, they would be required to undergo a health check and a 14-day quarantine (or self-quarantine);
✓ Restrictions on the entry of all tourists and foreign visitors into the country;
✓ Closure of all kindergartens, government and private schools including daily schools, boarding schools, international schools, tahfiz centres and other primary, secondary and pre-university institutions;
✓ Closure of all public and private higher education institutions (IPTs) and skills training institutes nationwide; &
✓ Closure of all government and private premises except those involved in essential services (water, electricity, energy, telecommunications, postal, transportation, irrigation, oil, gas, fuel, lubricants, broadcasting, finance, banking, health, pharmacy, fire, prison, port, airport, safety, defence, cleaning, retail and food supply.

3) JKM is one of the Frontliners as gazetted in The Approval Letter as Essential Services dated 29 March 2020 from Ministry of Health to Ministry of Women, Family and Community Development (KPWKM) under the same Act whereby the Minister of Health is clearly given authority to appoint any parties to implement the Act. Hence, Welfare Department (Jabatan Kebajikan Masyarakat) under Ministry of Women, Family and Community Development was appointed and officially become an Essential  Service / Frontliner. 

4) As a frontliner during MCO - we played significant role in helping people in need. This writing will focus on what Pejabat Kebajikan Masyarakat Daerah Hilir Perak / Bagan Datuk play its role during this crucial period. It may differ from another states / districts.

5) We were officially ordered to manage a Call Center  / Operation Center and maybe also can call War Room whereby all JKM have to set up a room / place from top to button, from HQ level to State level and down to District level. We are at the bottom in implementing the Act during MCO. We received many calls asking helps in many ways because of the restrictions - many affected especially cash flow in hands.  All the data received everyday will be submitted to District and Land Office (DLO) to do cross- check to make  sure only eligible people are given aid (if from  JKM - we provide foods busket), any other agencies such as Baitulmal provide cash as well as food busket to help people in need especially in this unprecedented period.  The DLO acted as a Bank Data from all Aid Giver Agencies to avoid overlapping data (same people receive many aids).  We created a taskeforce and collaborated with Civil Defence Force (Angkatan Pertahanan Awam APM), RELA and Ministry of Domestic Trade and Consumer Affairs in District Level in distributing food busket to those who affected during MCO. In Hilir Perak and Bagan Datuk districts - We successfully distributed more than 3,000 packs of food busket to people in need. The collaboration among Agencies make the taskeforce possible and completely done within 2 months. Along the taskeforce - there are lots joys as well as sorrow moments hidden behind the scenes but let it be a loving memory.

6) Besides that, we also collaborated with NGO, NGI and Corporate Sector as a fasilitator in channeling their aids to people in need especially those affected during MCO. All contributors / donators must report or inform their intention to deliver aids to JKM a week before. We as a fasilitator must dealing with another agencies such as APM and RELA to make the process of distribution go smoothly and in line with Standard Operating Procedure (SOP). The SOP itself stated that everyone (NGO, NGI & Corporate Sector ) must refer  to PKMD before distribution take place, only 2 people from NGO NGI and Corporate Sector is allowed to join the distribution and APM / RELA is a responsibled agencies to distribute all donations. During all these times, my phone non-stop ringing asking permission to that purpose.

7) During this crucial period, there are many people asking for cash to buy baby milk, pampers and so on. This happen before the government of Malaysia make an announcement of Bantuan Prihatin Nasional (BPN) - All people suffered with no sufficient cash in hands. Fortunately, JKM come to give a hand by contributing some cash for them. The grateful moments were there. They the victims thanks us and appreciated as well make Frontliner rejuvenated!

7) JKM also is responsibled to provide foods  to those quarantined in home under Home Surveillance Order (HSO). The list of the victim was from District Health Office and it's private and confidential. We as JKM received fund for the taskeforce. As all known, Teluk Intan was classified as a red zone and the possibility of the spread was high and in District Level - A meeting so called Mesyuarat Jawatankuasa Pengurusan Bencana Daerah Khas Menangani COVID-19 was held every week just to discuss and to see the progress of COVID-19.

8) We also as JKM has to play role in contingency plan in distributing foods to those who are not in existing recipients (from operation centre). We were asked to deliver food basket to foreigners stranded at  the sea nearby Hutan Melintang, Bagan Datuk. They were from Britain and Thailand who stranded because their boat broke down. They were not allowed to leave the boat and must stay still to avoid the spread of COVID-19. Then, we as frontliner provided them a box of food as they could survive before the boat repaired and leave our sea.

9) Moreover, we also have to work as usual in taking care of elders, children and any other vulnerable group ( 10 focused group under JKM). Home domestic violence, violence on children rampant reported. All these happen due to depression, anxiety, stress and emotional disorders as the consequence of MCO. With proactive approach - We launched D
ASS Psysosocial Survey to see how stress, anxiety and depression to people in Hilir Perak. In fact, many people affected mentally and emotionally during MCO.

10) One of our focused group or JKM client is from victims in disaster (Mangsa Bencana). We have to manage and coordinate Pusat Pemindahan Sementara (PPS) / Temporary Transfer Center during MCO. We strive to give the best to Flood Victims in this crucial moments of COVID-19. There's been 2 times flooding in Hilir Perak since the implementation of MCO. The MCO is still running until 31 August 2020 - the possibility for us to work and to serve flood victims is unexpected and unpredictable. However, we enjoyed every tasks given on our shoulders and it was something great to be remembered. According to Majlis Keselamatan Negara Orders ( MKN20) / National Security Council - JKM is responsibled on Welfare Committee and the tasks are as the following:

✓ Coordinating The Temporary Transfer Center / PPS;
✓ Registering Victim of Disaster;
✓ Providing Foods 3 times a day to the victims; &
✓ Carrying out such activities to victims as well as to give psychology support through counselling approach.

11) With the hectic moments throughout the pandemic -  All the District Welfare Officer private contact  number revealed in public until it becomes a chained fake message through Whatapps nationwide. All officers have been disrupted non - stop from day to night. We realized that those who are in trouble especially during MCO while they have no money, not allowed to work and to run their businesses because all things shut down except essential services. And now, all JKM  officer suffered mentally from the disruption but we still do our job in serving the people of Malaysia. That's the big sacrifice!

12) 2020 is a historical year and will be remembered forever. Do our part as a citizen - obey government's order to stay safe! Thank you all Frontliners.

📌Tiada buffet Ramadan di hotel-hotel seperti tahun-tahun sebelumnya. 

📌Tiada sambutan Hari Raya Aidilfitri, tidak boleh balik kampung, tiada kunjung mengunjung.

📌Tiada bunyi mercun, tiada yg cedera akibat mercun. 

📍 Harga minyak jatuh sehingga RM1.25/liter

📍Semua pejabat di seluruh Malaysia diarah tutup sepanjang Perintah Kawalan Pergerakan (PKP) dan kerja - kerja dilaksanakan sepenuhnya dari rumah.

📍Semua sekolah ditutup dan pendidikan dilaksanakan di atas talian. Kerja sekolah untuk murid dihantar dlm group what'sapp. 

📍Penjarakkan sosial sentiasa diutamakan. Bersalam tangan ditukar dgn meletak tgn di dada. 

📍Pita pelekat dilekatkan di atas lantai - lantai kedai untuk menentukan jarak selamat 1 meter di antara pelanggan. Beratur hingga berpuluh meter ke luar kedai. 

📍 Restoran dan kedai-kedai makan ditutup, hanya dibenarkan bungkus dan dibawa pulang sahaja. Food rider ibarat cendawan tumbuh lepas hujan. Ada yg kantoi jual rokok seludup dan ketum. 

📍Hanya beberapa orang sahaja dibenarkan berada di dalam kedai.

📍Kedai bukan keperluan asas dan perniagaan tidak berkaitan pembekalan makanan diarah tutup.

📍Keseluruhan acara dan musim sukan di seluruh dunia dibatalkan serta merta. Aktiviti riadah dan bersenam dalam rumah. 

📍 Konsert, perlancongan, festival, acara hiburan dibatalkan. Kawasan tumpuan di bukit bintang sunyi sepi yg sblm ini sesak dan padat 24 jam. Ibarat tiada waktu tidur. 

📍Majlis perkahwinan dan pelbagai sambutan majlis bersama keluarga dibatalkan. Tp diganti dgn nikah online. 

📍Tiada lagi jemaah di masjid dan rumah-rumah ibadat ditutup.

📍Tidak dibenarkan perkumpulan 50 orang atau lebih, kemudian diubah kepada 20 org atau lebih, hanya dibenarkan 10 orang atau kurang dari itu juga tidak dibenarkan.

📍Taman permainan kanak-kanak ditutup.

📍Kekurangan stok penutup muka, gaun,
 dan sarung tangan untuk kegunaan pekerja barisan hadapan.

📍Kekurangan alat bantuan pernafasan
 bagi pesakit kritikal.

📍Berlakunya pembelian panik sehingga kekurangan alat pembasmian, tiada sabun cuci tangan, tiada cecair pembersih tangan

📍Rak-rak di kedai menjadi kosong.

📍Kilang-kilang pembuatan, kilang penyulingan dan perniagaan lain telah menukar perkhidmatan mereka dengan membantu menyediakan visors, penutup muka, cecair pencuci tangan dan PPE.

📍Kerajaan telah melaksanakan sekatan jalan di setiap kawasan dan tertutup kepada pergerakan yang tidak penting.

📍Tiket kapal terbang murah, namun kita tidak boleh keluar melancong.

📍Sidang media dari Kementerian Kesihatan di keudarakan setiap hari.

📍Kemaskini kes baru, yang telah sembuh dan pesakit yang meninggal dunia dilakukan setiap hari jam 5 ptg. 

📍 Jalan raya menjadi kosong sunyi sepi tanpa kelibat manusia dan kenderaan selepas jam 8 mlm. 

📍Ramai orang memakai penutup muka dan sarung tangan apabila keluar.

📍 Pekerja perkhidmatan penting menjadi takut untuk keluar rumah.

📍Pekerja kesihatan pula, menjadi takut untuk pulang ke rumah dan berjumpa keluarga.

📍 "Mendatarkan lengkung graf" itulah yang sering dikatakan, bersama dengan kata-kata " Kita lakukan ini bersama" dan "Kita harus menang"
